Program is a specialized service for inpatients of the hospital with serious illnesses. These services are focused on providing patients with relief from the symptoms, pain, and stress of a serious illness-whatever the diagnosis. Services are offered by a multidisciplinary care team including doctors, nurse practitioners, nurses, social workers and chaplains to address patient or family needs. Services may include: Help with symptom management such as pain, nausea, shortness of breath and anxiety Patient and family meetings to assist with understanding of illness and goals of care Social services support Spiritual support
Program is a clinic that provides the following: Board-certified anesthesiologist, fellowship trained in interventional pain management Specializes in interventional pain management for a wide range of neurological-related conditions including including joint pain, neck and back pain, sciatica, postoperative pain, neuropathic pain, migraines and more Holistic view to provide personalized pain relief Variety of treatment options including nerve blocks, physical therapy, electrical stimulations, surgery and psychological support
Multidisciplinary program that specializes in the treatment of individuals who have chronic, severe pain of organic origin that has not responded to medical or surgical therapy. Treatment is individualized and may include occupational therapy, muscle relaxation, behavior modification, stress management, biofeedback, psychological and psychosocial counseling, nutrition counseling, food allergy testing, exercise programs and a variety of physical intervention techniques including electrical stimulation of selected peripheral nerves and laser bio-stimulation.
Program is an in-home service designed to help with quality of life for those being treated for chronic diseases. Services are offered by a care team including a nurse practitioner, social worker and chaplain to address any physical, emotional or social needs the patient or their family may experience. Services offered include: Medication list management, simplification, usage and potential side effect education Condition education Home safety check Help finding resources such as aides or financial assistance Support with healthcare decisions Communication with doctor
