Supported Living Apartment Program

Qualified staff is on-call twenty-four (24) hours per day and provide guidance and training in basic life skills focusing upon normalization and independent living skills during the waking hours.Progress is monitored with the development of an individualized program plan and through measurable outcomes that teach self development, decision making, and home management skills.Each complex has an office, laundry facilities, and a Community Room. The individual lives in a private apartment with rent assistance. The resident must possess personal skills to enable self-sufficiency. Program provides some daily living and skills training.

Active Treatment encompasses the social, emotional, cognitive, physical and communicative skills required for developing independent functional community living settings. The adult individuals served must be eligible for institutional treatment and must have a Developmental Disability as their primary diagnosis.

Supervised Living Program

This is a residential-based program offered in a group home setting. Residents who have limited skills receive ongoing training to enable them to move to more independent living. Training provided includes daily living activities, environmental and behavioral strategies, appropriate social skills, health and fitness, community integration, recreation and leisure interests, academic skills including signing, writing, basic math skills, fine and gross motor skills, and independent living skills such as time and money management, transportation training, domestic tasks, problem solving skills, and personal hygiene. Direct care staff strive to be unobtrusive, and positive with each resident.

Community Living Arrangement

Community Living Arrangement (CLA) is available for individuals who qualify for an intermediate level of care, where they can live in a smaller home-like setting. UCP provides training in areas such as self-care, daily living skills, money management, and community safety/survival skills. Individuals also benefit from recreational and leisure activities with the facility staff.
